The Matagorda County Hospital District’s project aims to increase WIC awareness and participation among families with children, seniors raising grandchildren, and pregnant teens in rural Texas.

The project will conduct outreach, test transit vouchers as a way to help ensure attendance at WIC appointments, and develop an electronic screen for primary care providers that triggers automated WIC referrals.

Lessons learned from this project will include best practices on improving collaborations between WIC and healthcare providers, such as pediatricians, population health nurses, and other clinicians who routinely assist patients in rural communities.
